Terms of Use
These terms apply when you use the UniversalHX Personal Record and when you connect a health system to it. Please read them before you connect a record. How we handle your information is described in our Privacy Policy.
What this service is
The UniversalHX Personal Record ("UHX") is provided by Axella Research LLC, a New York limited liability company ("we", "us"). It is a personal health record. It lets you connect the health systems where you are a patient and keep your own copy of the information they hold about you, in one place that you control.
It is a record-keeping tool. It is not a medical device, it does not provide medical advice, and it is not a substitute for talking to a clinician. Nothing here should be used to make a treatment decision on its own, and it is never a route to emergency care. In an emergency, call your local emergency number.
Who can use UHX
You must be 18 or older to create an account. A parent or legal guardian may manage a child’s record from their own account and is responsible for that use; when the child turns 18, the record can be handed over to them.
You agree to these terms and to the Privacy Policy when you create an account, or when you continue after we ask you to agree to a new version.
AI features
Some features use artificial intelligence: the assistant, reading a document you scan or upload, summaries, health trends and intake suggestions. They are off until you allow them, and you can turn them off at any time in Settings, under AI and your data, where every AI use is listed.
When you use one, the part of your record that feature needs is sent to Anthropic, which provides the AI model, to produce the answer. It is not used to train AI models, by us or by Anthropic.
AI output can be incomplete or wrong. It is not medical advice and is not reviewed by a clinician before you see it. Check anything important against the original record and with your clinician.
Health privacy laws
For the record you keep for yourself, we are not acting for your doctor or health plan, so the federal HIPAA law generally does not apply to the copy you keep in UHX. Your records at the hospitals and practices that hold them remain covered by HIPAA and by their own policies.
We protect your copy as the Privacy Policy describes, we never sell it or use it for advertising, and the Federal Trade Commission’s Health Breach Notification Rule requires us to tell you if it is ever exposed in a breach.
What we read from a health system you connect
When you connect a health system, you are taken to that health system to sign in. We never see your portal username or password. You choose whether to approve the connection, and the health system tells us what you approved.
With your approval we read: your demographics, visits, conditions, medications, allergies, immunizations, procedures, care plans, care team, insurance coverage, lab and diagnostic results, and clinical documents and notes.
We request read access only. The app has no ability to write to, change, or add anything in your health system record. Information flows one way: from them, to your copy here.
What we do with it
We use it to build and show your record, and to power the features you choose to use inside the app.
We do not sell your health information. We do not use it for advertising, and we do not share it with anyone for their own marketing.
Who else can see it
Nobody, unless you decide otherwise. Your record is visible to you, and to anyone you explicitly share it with.
You can share a document, a section, or access to your record with a clinician or a family member. Every share is something you create, and every share is something you can withdraw.
Messages and your conduct
Messages lets you write privately to the practices and people you are connected with on UHX. Anyone you are not connected with has to send a request first, which you can accept, decline or block.
Do not use UHX to harass, threaten, deceive or impersonate anyone, to send spam, or to share sexual, hateful, violent or unlawful content. We do not tolerate objectionable content or abusive users.
You can block anyone, and report them from the conversation. We review every report within 24 hours and remove content, and close accounts, that break these rules.
Disconnecting and deleting
You can disconnect a health system at any time. Disconnecting stops any further reading from that health system immediately.
When you disconnect you choose what happens to what was already imported: keep your copy, or delete it. You can also delete your account, and the record with it, from inside the app.
Keeping it secure
Connections are made over an encrypted connection, and the credentials that let us read on your behalf are stored encrypted and are never shown to you or to anyone else.
No system is perfect. Use a strong, unique password, keep your second factor enabled, and tell us promptly if you think someone else has reached your account.
Accuracy
What you see here is a copy of what a health system sent us. If something looks wrong, the authoritative record is the one held by the health system or practice that created it, and corrections have to be made with them. We can show you what we received; we cannot change what they recorded.
Changes to these terms
If these terms change in a way that affects what we read, who can see it, or what we do with it, we will say so in the app before the change takes effect, and ask you to agree to the new version before you continue.
